1) What benefits am I entitled to as
a new Teacher with the Toronto Catholic District School Board?
As a permanent Teacher with the TCDSB
you are eligible to participate in Health, Dental and Life Insurance plans.
Our benefits are provided through Great-West Life Insurance.
2) Is Health and Dental benefits available
to me immediately or is there a waiting period?
You are eligible for Health and Dental
benefits immediately. However, since coverage is optional, it is
not valid until you have completed the Benefits enrolment form. Coverage
commences on the first day of the month following receipt of the enrolment
form in the Benefits Department.
3) Is there a cost to me in order to
receive these benefits?
Depending on your Collective Bargaining
Agreement, there may be a monthly cost associated with Health and Dental
coverage.
4) What does the Extended Health Care
plan cover?
· Vision care $150 every two calendar
years (laser eye surgery is not covered)
· 100% prescriptions reimbursed
with no annual maximum [following payment of an annual deductible of $10(single
coverage) and $20(family coverage)]
· Semi-private hospitalization
coverage
· Out-of-Province coverage reimbursed
at Ontario rates, however, there is No-out-of-Country coverage available
· $7.00 dispensing fee cap currently
in force

5) Does the Dental Plan cover 100% of
incurred expenses?
Currently claims are reimbursed at the
1995 Ontario Dental Association Fee Schedule
· 100% of costs of basic dental
work
· 50% of costs of major and orthodontic
work with a $3000 orthodontic, and $10,000 Major Restorative lifetime caps
6) What family members qualify as dependants
of an employee?
A dependant means:
· your spouse (the person of the
opposite sex to whom you are legally married)
· your unmarried child under 21
years of age and dependant on you for support
7) What is the Group Life Insurance
Policy?
Participation in the Group Life Insurance
plan is mandatory for all permanent employees of the Board. The policy
is payable upon death to designated beneficiary in the amount equal to
three times your annual salary. The monthly premium cost varies depending
on your salary.
8) What is Optional Life Insurance?
You may qualify for additional life insurance
coverage through the Board at Group rates. Enrolment is voluntary
and premium rates are based upon age, sex and whether or not you smoke.
9) How are claims processed?
Anyone enrolled in the Extended Health
Plan will receive a plastic I.D. card (two for family coverage).
This card can be used to facilitate direct payment of drug claims at the
pharmacy. Request for prescription reimbursement, as well as for
other medical claims, can be submitted via claim forms directly to Great-West
Life. Claims forms are readily available from all school locations
as well as through the Benefits’ Department.
